چکیده

Mitral regurgitation (MR) after myocardial infarction is a relatively common complication and its incidence is about 50%. One cause of MR after myocardial infarction is papillary muscle rupture, and this situation usually requires urgent surgical treatment. Since prognosis is favorable with early surgical intervention, early diagnosis of the underlying cause of mitral regurgitation is very important. Transthoracic echocardiography is an excellent tool for detection and estimation of the severity of mitral regurgitation in patients with myocardial infarction, especially those who have a systolic murmur. But transthoracic echocardiography may have limitations for detailed exploration of the subvalvular apparatus of the mitral valve. Multiplane transesophageal echocardiography (TEE) has been used for accurate evaluation of the mechanism of mitral regurgitation in patients with acute myocardial infarction. We experienced a case of severe mitral regurgitation due to papillary muscle rupture that was confirmed only by transgastric view of TEE.

A rare complication of AMI is papillary muscle rupture (PMR), occurring in 1% of patients with acute myocardial infarction (AMI). Mitral regurgitation related to PMR occurs at a median of 1 day (ranging from 1-14 days) after onset of myocardial infarction. PMR tends to occur in those having a first time AMI and those without extensive collateral circulation. PMR involves posteromedial papillary...

Chronic Papillary Muscle Rupture: 14-Year Survival without Surgical Treatment.

UNLABELLED Papillary muscle rupture is a life-threatening complication of myocardial infarction which is usually refractory to medical treatment. We present a very rare case of a 65-year-old woman who had a myocardial infarction and posteromedial papillary muscle rupture which was only treated with medical therapy, including her corresponding 14-year follow-up. However, surgical intervention is...

Triple cardiac rupture.

Left ventricular free wall rupture and acute ischaemic mitral regurgitation are nowadays rare, but still potentially lethal mechanical complications after acute myocardial infarction. We report a case of a sequential left ventricular free wall rupture, anterolateral papillary muscle disruption, secondary severe mitral regurgitation and subsequent posteromedial papillary muscle head rupture in a...

A forgotten devil; Rupture of mitral valve papillary muscle

BACKGROUND Papillary muscle rupture is one of the catastrophic mechanical complications following myocardial infarction. Rupture leads to acute mitral valve regurgitation, pulmonary edema, and cardiogenic shock. Survival is dependent on prompt recognition and surgical intervention. CASES REPORT We present two cases where acute myocardial infarction was complicated by papillary muscle rupture ...

Surgical treatment of complete anterolateral papillary muscle rupture following acute myocardial infarction.

Papillary muscle rupture is a lethal complication of after acute myocardial infarction. We report the successful surgical management of complete anterolateral papillary muscle rupture associated with cardiogenic shock in an 80-year-old woman. The patient was admitted with the sudden onset of chest pain and dyspnea. After hemodynamics deteriorated rapidly, she was intubated and intra-aortic ball...
